Nicotinamide adenine dinucleotide (NAD+) is the central redox coenzyme of every living cell and the obligate substrate for the sirtuin (SIRT1–7) and PARP enzyme families. Tissue NAD+ falls roughly 50% between young adulthood and senescence — one of the metabolic hallmarks of ageing — as the NAMPT salvage pathway slows and CD38 consumption climbs. Orally administered NAD+ is almost entirely hydrolysed in the gut, with under 5% reaching circulation intact; the subcutaneous lyophilised format bypasses first-pass gut metabolism to present the intact coenzyme directly to the sirtuin and PARP axes — the reason parenteral delivery has become the format of interest in NAD+ research.
